your discharge from physical therapy.
What does a free 15 minute orthopedic
screening entail?
An
orthopedic screen is a free service provided to you by a licensed
physical therapist to quickly answer any questions you may have
about your physical health, injuries, or symptoms. We will provide
a brief screen and provide a recommendation for treatment and/or
follow up care. If necessary, we may recommend you see a physician,
specialist or schedule a full physical therapy visit for a more
comprehensive physical therapy diagnosis and recommended treatment.
Do you accept my insurance?
Peak Form Physical Therapy is contracted with most major insurance
networks and carriers. Please see our list
for current accepted insurance networks and carriers. If you do
not see your insurance carrier listed, please call our office
to confirm. Our front office staff is more than happy to verify
your out-patient physical therapy benefits through your insurance
carrier prior to scheduling so you are aware of number of visits
allowed, co-pay or co-insurance and/or limitations within your
policy.
What if I do not have insurance coverage?
No insurance, no problem. If you do not have health insurance
coverage or in some cases, have a high co-pay or co-insurance
amount, we do offer a private pay option for physical therapy
sessions. We offer a 50% discount should you chose to pay privately.
To qualify for this discount you must pay at the time of service.
It is difficult for us to provide an estimate of costs until your
physical therapist performs an evaluation and determines what
treatments are appropriate.

How long will my visits last?
Your first appointment will last approximately 60 minutes. Your
following appointments will last approximately 30 to 45 minutes
depending on your condition and treatments.
What should I expect during physical
therapy?
Your
physical therapist will review your medical history and perform
a physical evaluation. Depending on your condition, your therapist
may evaluate your strength, range of motion/flexibility, endurance,
balance, coordination, posture, walking and stair negotiation,
and body mechanics. Please wear appropriate clothing (ex: t-shirt,
shorts, tennis shoes, sports-bra). Your therapist will then discuss
with you their findings and suggested treatment plan. You will
also develop goals for physical therapy together. You will most
likely be given a home exercise program that includes specific
pictures and instructions for exercises.
Will you communicate with my physician
regarding my physical therapy?
Yes. We immediately fax your initial evaluation to your physician.
We also re-assess your condition every 10 visits or after 30 days
of treatment and write a progress note documenting your progress.
We also send this note to your physician. Please let us know when
your follow up appointment is with your physician, and we will
prepare a progress note to keep them up to date on your current
status.
Do I need a prescription to receive physical
therapy?
No. Colorado is a “direct access” state and you do not need a
prescription from your physician to receive out-patient physical
therapy. However, some insurance plans do require you to have
a prescription from your physician in order for them to pay for
physical therapy services. Feel free to call our office and we
can verify your insurance requirements. Our physical therapists
are highly trained health care professionals and are well qualified
to evaluate a patient's condition, assess his or her physical
therapy needs and, if appropriate, safely and effectively treat
the patient. Physical therapists recognize when it is appropriate
to refer patients to other health care professionals for consultation
if needed.
