About Us
Peak Form Physical Therapy LLC, Boulder, CO
was founded in August 2003 by physical therapist Jeanette
Hrubes and the grand opening of the Peak Form Physical
Therapy clinic was in November 2003. After nearly three years
in business, Peak Form is a bustling Physical Therapy clinic with
a large referral network and loyal clientele.
Jeanette
Hrubes, PT / Owner, Director of Boulder and Brighton Clinics
Jeanette originally hails from lovely British Columbia, Canada.
She made her way to the United States on a gymnastic Collegiate
Athletic Scholarship, attending the University of Florida (Go
Gators!). She earned her Physical Therapy Degree in 1994 and remained
in Gainesville, Florida; working as the senior physical therapist
in a bustling orthopedic/sports medicine clinic. She was eventually
drawn back to the Rocky Mountains, moving to Boulder in 2002.
She then opened her own patient-focused physical therapy clinic
a year later. Although she has retired from competitive gymnastics,
she is still faithful to the active lifestyle,…getting her fix
through many outdoor activities including; Ultimate Frisbee, soccer,
mountain biking, rock climbing and grueling adventure races. Best
of all, Jeanette is the proud parent to a little athlete in training.
Jill
Gum, PT, DPT, ATC, CSCS / Senior Physical Therapist, Brighton
Clinic
Dr. Jill Gum is a licensed physical therapist, certified athletic
trainer (ATC) and certified strength & conditioning specialist
(CSCS). She graduated from Regis University in Denver, Colorado
with a Doctorate of Physical Therapy (DPT) degree in 2007 and
has also earned a Bachelors of Science degree in Athletic Training
in 2003 from Creighton University located in Omaha, NE. Her focus
in physical therapy lies in combining therapeutic exercise and
patient education with manual physical therapy to provide effective
rehabilitation interventions. Dr. Gum also specializes in aquatic
physical therapy and utilizes the pool at the Brighton Recreational
Center as an effective compliment to her therapy regimens.
Dr. Gum enjoys treating patients of all ages for orthopedic, sports,
and work injury rehabilitation. Born and raised in Omaha, Nebraska
(and no she is not a Husker fan!), she moved to Colorado to attend
physical therapy school. She enjoys spending her free time with
the men in her life, her husband, Ryan, who is also a physical
therapist & their dog, a Weimaraner named Brutus. They love
cooking Italian food, watching movies, skiing and hiking in the
Rockies, and traveling! Dr. Gum is an active member of the American
Physical Therapy Association, National Athletic Trainers Association,
& National Strength & Conditioning Association.
Kim
Edwards, PT, DPT / Staff Physical Therapist, Brighton Clinic
Kim recently moved to Brighton from Memphis, TN. In high
school she competed in cross country, track, soccer and swimming.
She attended Harding University in Arkansas for her undergraduate
degree and earned her BS in Exercise Science. A knee injury prevented
her from competing in college, but her love for sports and her
interest in the human body and how it works helped her develop
her love for the field of physical therapy. Kim then attended
the University of Tennessee in Memphis, where she earned her Doctorate
of Physical Therapy degree.
She and her husband enjoy spending time outdoors hiking, biking,
skiing and exploring Colorado. She also enjoys training for the
occasional triathlon or adventure race. Her other hobbies include
cooking, baking, and creating new dishes for her friends and family
to enjoy.

Stacia
Roberts, CMT / Certified Massage Therapist, Boulder Clinic
As a Colorado native, Stacia was blessed with the opportunity
to grow up camping, backpacking and cross country skiing. She
still enjoys these activities and spends her free time hiking
in the high country. Her love of new experiences and adventure
enticed Stacia to spend much of her 20’s traveling. Her destinations
included Nepal, China, Thailand, Sumatra and Central America.
The birth of her daughter curtailed her freedom to traipse across
the globe, but began the next, equally exciting journey through
parenthood.
Stacia and her almost-grown daughter have been living in Boulder
for the past twelve years where they enjoy the proximity of family,
open space and the incredible offerings of the Rocky Mountains.
They feel it is a true blessing to be living in the unique atmosphere
of The Republic of Boulder.
Stacia is a 1988 graduate of the Boulder School of Massage Therapy
and has extensive injury rehabilitation experience. She has a
dynamic continuing education resume which focuses on treatment-oriented
massage modalities, including a certification in neuromuscular
therapy. She has also taken multiple other trainings with content
varying from body-centered psychotherapy to leadership. She is
also a recent graduate of the Biology Department of CU Boulder
where she focused on human physiology.
Stacia took over the responsibilities of Massage Director at Mountain’s
Edge Fitness Center in June 2007. She has been busy remodeling,
redeveloping and redirecting their Massage Clinic into a vibrant
department with a contingent of talented therapists.
Andrea
Kidd / Office Manager, Boulder and Brighton Clinics
Andrea is a rare Colorado native. She grew up in Estes Park, where
she graduated from high school. Soon afterwards she decided to
move to Steamboat Springs to do a little snowboarding and attend
Colorado Mountain College. After Steamboat she moved to Houston,
Texas, where she withstood the humidity for four years before
returning to her home state of Colorado in 2002.
Prior to employment with Peak Form Physical Therapy, Andrea spent
five years running and managing her family’s business in Lyons,
Colorado. After serving the local community for seventeen great
years, the family decided to sell the business in June 2007. After
taking the summer off to spend time with her sister and her kids
from Texas, she finally decided that she had played long enough
and landed a job with Peak Form Physical Therapy. Even though
Andrea was new to the medical field she was a quick learner and
soon assumed the position of Office Manager.
Andrea enjoys traveling and is excited to try new things and places.
Having been to England, Austria, Holland, Belgium, France, Germany,
Italy, Jamaica, Australia, and Mexico, she hopes to someday retire
in Italy, or at least be able to go there often. When Andrea is
not at work, she enjoys being with her family and friends. She
takes pleasure in hiking, snowboarding, skydiving and scuba diving,
and she is always up for a new adventure.
Jessica
Theriault / Front Desk, Brighton Clinic
Jessica joined Peak Form Physical Therapy in September 2007 and
really enjoys meeting all the new people. Jessica is in her second
year of college and is majoring in Criminal Justice. Her goals
are to become an FBI Agent or Homicide Detective, which explains
another interest: Court TV!
Jessica has resided in Brighton, CO for the past fourteen years.
While she was born in the south, and is her mom’s “Southern Belle,”
she has traveled throughout the US. Traveling is an addiction
not easily quit and she hopes to pass it on to her favorite traveling
partner, her two-year-old daughter. They’ve already toured California,
Maine, New York and New Mexico together and have many more trips
planned in the future. When stuck indoors, Jessica loves dancing,
singing, and reading, but as soon as summer hits, you’ll find
her outside camping, hiking, river rafting, or simply out for
a walk.
Emily
Hansen / Front Desk, Physical Therapy Technician, Boulder and
Brighton Clinics
Emily was born in California, but grew up in Austin, Texas, where
she graduated high school in 2002. The mountains lured her to
Colorado and she decided to do her undergraduate work at CU. After
two long, boring years as a Biology major she switched to Integrative
Physiology. She graduated in December 2007 with a bachelor's degree,
and she is currently in the application process to attend physical
therapy school in the Denver area. It is her goal to graduate
with a DPT and continue on as a physical therapist in the great
state of Colorado.
To relax she enjoys reading, playing the flute, knitting, and
watching movies. Some of her favorite activities are yoga, step
aerobics, and hiking. She loves exploring the Denver area with
her dog, Merlin - he's always by her side, and if you visit the
office on the right day you may get to meet him! She loves to
travel to new places and has been lucky enough to visit France,
Italy, Mexico, and Hawaii, although her ultimate vacation would
be a flight to outer space. She also helps her mother on her alpaca
ranch, so feel free to ask questions if you're curious!
Suzi
Carlson / Billing Coordinator, Boulder and Brighton Clinics
Suzi was born in eastern Colorado where she grew up with her family
and graduated from high school in 1995. Coming from a small farming
community she ventured to the "Big City" of Longmont
where she met and married her better half. They moved to Platteville,
another small farm community, to start their life together. They
became a family with the addition of a daughter, Sami, in the
fall of 1996.
Suzi stayed at home and raised their little munchkin until she
began pre-school in the fall of 2000. Upon this turning of the
page she decided to get a job working at a clinic in Longmont.
She trained there and was then sent to school in Dallas, TX to
become efficient in insurance billing and coding. Suzi started
her own billing service in 2003, billing for chiropractic and
physical therapy clinics. She has now been with Peak Form Physical
Therapy for two years and is proud to continue to serve the clinic
and the patients’ needs in the insurance field.
When not arguing with the insurance companies, Suzi enjoys spending
her time with her husband Jeff and daughter Sami. They love camping,
fishing, golfing, and spending time with their extended family.
She also loves to coach volleyball for the middle school and recreation
center in her community. She's still an avid player herself playing
on women’s and co-ed competitive teams.
