About Us

Peak Form Physical Therapy LLC, Boulder, CO was founded in August 2003 by physical therapist Jeanette Hrubes and the grand opening of the Peak Form Physical Therapy clinic was in November 2003. After nearly three years in business, Peak Form is a bustling Physical Therapy clinic with a large referral network and loyal clientele.

Jeanette Hrubes, founder of Peak Form Physical Therapy, Boulder, COJeanette Hrubes P.T. /Clinic Director originally hails from lovely British Columbia, Canada. She made her way to
the United States on a gymnastic Collegiate Athletic Scholarship, attending the
University of Florida (Go Gators!). She earned her Physical Therapy Degree
in 1994 and remained in Gainesville, Florida; working as the senior
physical therapist in a bustling orthopedic/sports medicine clinic. She was
eventually drawn back to the Rocky Mountains, moving to Boulder in
2002. She then opened her own patient-focused physical therapy clinic a year later. Although she has
retired from competitive gymnastics, she is still faithful to the active
lifestyle,…getting her fix through many outdoor activities including;
Ultimate Frisbee, soccer, mountain biking, rock climbing and grueling
adventure races. Best of all, Jeanette is the proud parent to a little
athlete in training.

Kerry Barnhold, Physical Therapist with Peak Form PT, Boulder, COKerry Barnholt P.T.;D.P.T / Senior Physical Therapist, is a 2004 graduate of the Regis University Doctor of Physical Therapy (DPT) program and brings to Peak Form Physical Therapy diverse experiences that have helped shape her skills as a therapist.
Originally from northern California, Kerry grew up with a passion for athletics and a curiosity for adventure. She was a competitive gymnast through her youth and, upon graduating from high school, attended the University of California Davis where she competed as a NCAA springboard diver. She graduated in 1996 with a bachelor of science in Physiology and then moved to the mountains of Lake Tahoe where she worked on the ski patrol at Squaw Valley ski area.
Upon moving to Boulder, Kerry pursued a master’s degree in Kinesiology from the University of Colorado, Boulder and subsequently a physical therapy degree from Regis University. During her years in Boulder, Kerry has become an active member in the local cycling community. While working part time as a physical therapist, Kerry raced mountain bikes professionally for the Subaru Gary Fisher mountain bike team but decided to “retire” from professional racing in 2005 and she has been working for Peak Form Physical Therapy ever since.

Heather Meyer P.T.; D.P.T./ Staff Physical Therapist graduated in 2005
from Concordia University Wisconsin with a Doctor of Physical Therapy
degree (DPT). After graduation she began work in an outpatient setting
gaining experience treating patients with either orthopedic or
neurological conditions.
Heather grew up in Kansas City, Missouri until leaving to attend
undergraduate classes at Concordia University Nebraska. Throughout her
life Heather has been involved in various sports including Volleyball,
Basketball, and Softball. While at Concordia Heather was a member of the
college Softball team. In 2002, she graduated with a Bacherlor of Science
degree in Biology with a minor in Chemistry.
Having just recently moved to Boulder, Colorado. Heather started work at
Peak Form Physical Therapy in August of 2006. She is excited to get to
know people in the area and put her skills to use. In her spare time
Heather has enjoyed hiking, bike riding and exploring the beautiful
outdoors with her husband. She is looking forward to skiing this winter.

Toby Schaid MSPT; FAAOMPT Originally from the Dallas, Texas area, Toby has recently moved to Boulder from Eugene, OR. Toby has always been active and, while growing up, she was involved many sports including gymnastics, volleyball, softball, and track and field. She is always up for trying something new and has recently been enjoying mountain biking, skiing, and snowboarding. Toby attended the University of Kansas, in Lawrence KS and while earning her degree in Sports Science she was an active member of the successful Women’s Lightweight Rowing team. She graduated in 1995 with a B.S. in Sports Science and then continued on with her schooling to pursue a master’s degree in Exercise Physiology but she ultimately switched to Physical Therapy and then graduated from the University of Kansas Medical Center in Physical Therapy in 1998. After graduation Toby relocated to Eugene, Oregon and began working for a community hospital system. In 2004, she attended a Fellowship in Advanced Manual Physical Therapy in the Bay Area CA and in 2006 she became a Fellow in the American Academy of Orthopedic Manual Therapists. Since late 2004, Toby has been balancing parenthood with her physical therapy career and spends most of her free time trying to keep up with her active little boy.

Amy Jaffe, staff at Peak Form PtAmy Jaffe / Office Manager joined the Peak Form Physical Therapy staff in April 2006 after staying at home the last ten years to raise her son and daughter. She has worked many years in the medical community and obtained a B.A. in psychology from Allegheny College in Pennsylvania. Amy has always dabbled in health and taught aerobics locally for fifteen years. Additionally, she has also worked in retail since being raised in the family business. Amy also has three older daughters who are learning how to survive in Las Vegas.

A rare Boulder native, Amy enjoys many activities, including swimming, hiking with her dogs, golf, jumping on the trampoline and playing basketball with her kids. If she has a spare minute, she can usually be found reading a book or a magazine. Stop by the front desk and discuss Boulder then and now with her!
