Welcome to Peak Form Physical Therapy. We are a physical therapist owned, independent, out-patient physical therapy practice specializing in orthopedic rehabilitation. We have two locations: Boulder, CO and Brighton, CO.

Following discharge from formal physical therapy, we offer a “Step Up Program” to all patients at both locations to maintain your improvements. You will continue to have access to our gym, equipment, and staff.
